Hall of Fame Nomination Form


A. Hall of Fame candidates are selected from the communities of Fountain Hills, Fort McDowell or the Verdes.
B. A candidate must have resided in or been active in one of these communities at least 10 years prior to nomination.
C. Candidates are judged on their past and recent activities. Candidates are selected from the following categories: Community Service, Education, Governmental Activities, and Religious & Spiritual Activities.
D. A candidate may be nominated by anyone.
E. The candidate may be living or deceased.

Nomination forms must be submitted no later than December 29 of each year.
